Nota informativa para coordinadores de la Feria Comunitaria de la Salud
A nombre de AHPSI y del Desfile y Festival Dominicano del Condado de Union, informamos a todos los coordinadores, agencias, profesionales, terapistas y voluntarios participantes sobre la importancia de utilizar correctamente los tres formularios básicos durante nuestra Feria Comunitaria de la Salud “Decide to Be Healthy”.
Estos formularios son herramientas esenciales para mantener una feria organizada, documentar los servicios ofrecidos y facilitar el seguimiento de los participantes cuando sea necesario.
Los tres formularios principales son:
1. Registro General del Paciente y Servicios a Ofrecerse
Permite identificar al asistente, conocer qué servicios necesita y orientarlo adecuadamente dentro de la feria.
2. Hoja de Registro Interno por Área de Servicio
Debe ser utilizada por cada agencia, profesional, terapista o voluntario para registrar los servicios prestados y el total de pacientes atendidos en su área.
3. Hoja de Referimiento para Exámenes Anormales
Facilita orientar a los participantes que presenten resultados alterados o necesidades especiales, conectándolos con seguimiento médico, educativo o comunitario.
Solicitamos a todos los coordinadores asegurar que estos formularios sean usados correctamente y recopilados al final de la feria. Su buen uso fortalece la organización, la evaluación del impacto comunitario y la calidad del servicio que ofrecemos a nuestra comunidad.

Informational Note for Community Health Fair Coordinators
On behalf of AHPSI and the Dominican Parade and Festival of Union County, we would like to inform all coordinators, agencies, health professionals, therapists, and participating volunteers about the importance of properly using the three basic forms during our “Decide to Be Healthy” Community Health Fair.
These forms are essential tools to help keep the fair organized, document the services provided, and facilitate follow-up for participants when needed.
The three main forms are:
1. General Patient Registration and Services Form
This form helps identify each attendee, understand what services they are requesting, and guide them properly throughout the health fair.
2. Internal Service Area Registration Sheet
This form should be used by each agency, professional, therapist, or volunteer to record the services provided and the total number of patients served in their area.
3. Referral Form for Abnormal Screening Results
This form helps guide participants who present abnormal results or special needs, connecting them with medical, educational, or community follow-up services.
We kindly ask all coordinators to ensure that these forms are used properly and collected at the end of the fair. Their proper use strengthens the organization, evaluation of community impact, and quality of service we provide to our community.
