The licensure fee for the New York State Laboratory License Exam is $345. This fee is non-refundable. You can pay the fee online or by mail.
To pay online, go to the NYSED website and click on the “Pay a Fee” tab. Select the “Clinical Laboratory Technologist” license and enter your application information. You will then be able to pay the fee with a credit card or debit card.
To pay by mail, download the “Licensure Fee Payment Form” from the NYSED website and mail it along with a check or money order made payable to the “New York State Education Department” to the following address:
New York State Education Department Office of the Professions 625 Broadway Albany, NY 12234-0001
Please note that you must pay the licensure fee before you can take the exam. You can find more information about the New York State Laboratory License Exam on the NYSED website.
